The application consists of a copy of the Intent to Participate contract for each involved school district; a listing of schools participating in the planning; a Memorandum of Agreement to particpate in the school planning process; budget documents; meeting minutes; a list of school board members; biographical data sheets on each board member; bylaws of the school board; and recruitment brochure for a superintendent of the new school.
